    We still cannot surely say if problem is in hardware or software. On hardware side, swap monitor connectors, so that primary display connector goes to secondary monitor and secondary display connector goes to primary monitor. Also HDMI-DVI adapter may cause problems.

    Take VGA cable off from video card (turn off computer first) and monitor, try what happens then. Card may still detect VGA cable even if disconnected from monitor.
